Our Guest Services Associates at the Front Desk is ready to serve you from 8:00
am to 8:00 pm, Mondays to Sundays. You may call them at ext. 125 or 126. |

Prior to admission, you may wish to check out the facilities and services of the
hospital. For tour requests, call our Admission at ext. 110. |

For inpatient services, please report to Admission at the main lobby. The Guest
Services staff will escort you to your room. |
Our nurses will give you a Patient Perception Survey Form. Please complete the survey
form so we can better serve you, your families and friends. Kindly return the accomplished
form to the Nurse Station or to the Guest Service Department.
